Community Options

Community Options provides episodic case management for people who are frail aged, people with a disability, and their carers.  Our clients have multiple and complex needs and experience difficulty performing activities of daily living; and may be at risk of admission to residential care. 

Our case management service includes:

  • a comprehensive assessment of strengths and needs;
  • developing goals and a support plan in conjunction with the client (and carer if appropriate);
  • implementation of services;
  • facilitating community connections;
  • monitoring and reviewing the support plan;
  • information and referrals; and
  • advocacy and support.

We welcome a diversity of clients, including:

  • people from culturally and linguistically diverse backgrounds;
  • Aboriginal and Torres Strait Islander people;
  • people with dementia;
  • financially disadvantaged people; and
  • people living in remote or isolated areas. 

We can also support people with acquired brain injury,  chronic medical or neurological conditions, or mental illness.

ComPacks

ComPacks provides up to 6 weeks of co-ordinated services to assist a person return home safely after discharge from a participating NSW public hospital. Referrals are required to be completed by hospital staff.  These services may include:

  • personal care;
  • domestic assistance;
  • shopping;
  • transport to medical appointments; and
  • social support.

  • Home Modifications: service for people who are frail aged, people who have a disability and their carers to assist by constructing access ramps, installing grabrails, handrails and other bathroom modifications etc. 6492 3756
  • Karanya Uniting Care Community Care: services focus on enabling, empowering and promoting independence so people can remain active members of their community.   • Meals on Wheels Plus: Food services for frail aged or people with a disability, including home delivered meals, frozen food, centre-based meals at a minimal cost.
